Root Cause Analysis (RCA) is the use of a number of tools and methodologies to investigate the causes of a problem. Properly completed, an RCA will uncover all of the factors underlying a problem, help to identify other related risks and suggest appropriate solutions. The RCA methodology combines basic principles with a series of connected techniques in order to identify the factors that go into the successful performance of a process or system and, therefore, where factors can combine to cause a system to fail.
Where RCA differs from ‘normal’ investigation is that it doesn’t seek blame nor does it assume a single cause for any event. It goes beyond superficial cause and effect helping you to show how processes or systems failed or caused an issue in the first place and, most importantly, helping you to ensure that it doesn’t happen again!
